Basic Skills Minor Forms for 16 or 17 Year-Old Students

According to state statute NCAC 2C.0305(b), students 16 or 17 years of age are eligible for the Basic Skills Program provided a proper school release is obtained and a notarized petition of the student’s parent, legal guardian, or other person or agency having legal custody and control is submitted.

To enroll into the Roanoke-Chowan Community College Basic Skills Program, three minor forms must be completed. These forms can be obtained by contacting 862-1309, stopping by the Basic Skills Office in the Freeland Building Room 126, or by printing out the forms below.

These forms MUST be completed and brought to the first session of the Basic Skills Orientation for any student who is 16 or 17 years of age.
